Boarding, Daycare, Training and Grooming

Client Service Agreement and Release of Liability

This agreement includes a release of liability and waiver of legal rights and deprives you of the right to sue the owner(s) of Family Dog Care or related parties. By contracting any services of Family Dog Care you are agreeing to the terms and conditions of this Client Service Agreement and Release of Liability. Do not sign this agreement and/or contract Family Dog Care services unless you have read and understand it in its entirety. Seek the advice of legal counsel if you are unsure of its effect.

In consideration for your animal(s) (herein referred to as “Guests”) being permitted to come to Family Dog Care premises or use any Family Dog Care services, by accepting this Agreement you agree to all the policies, procedures, release, terms and conditions set forth below.

I, the Primary Pet Owner, understand that I place my pet(s) under the care of Family Dog Care for boarding, daycare, training, grooming, dog walking, pet sitting, wellness, transportation, and other services.

Family Dog Care shall exercise diligence, care and safety precautions for your pet(s) while in our care. Family Dog Care and its staff, owners, contractors, and agents collectively will not be held liable for loss or damage from disease, death, escape, theft or fire, and from injury or damage done by your pet to people, other animals, property, himself, allergic reaction to medication dispensed by owner or vet, or other unavoidable causes. Should your pet cause damage to another dog or the property, you as the owner will be responsible for those costs incurred. I hereby release Family Dog Care from any liability arising from my Pet’s attendance and participation in the day care, boarding, training group-play environment and any off-site play experiences.


Health and safety are of utmost importance to us at Family Dog Care. We do everything in our power to maintain a clean, safe, and healthy environment for our canine members. All canine members must be current on vaccinations, and provide appropriate documentation prior to participating in daycare, boarding or training. Pet(s) not up to date with required vaccinations as set forth with Family Dog Care will be turned away or refused services.

You agree to provide veterinary records to Family Dog Care upon request, and you hereby grant Family Dog Care permission to request and obtain veterinary records directly from your Designated Veterinarian. If Family Dog Care is unable to obtain necessary records from you, your pet will be denied services. Client represents and warrants that pet(s) are currently vaccinated in accordance with all local, state and federal laws and regulations. Client agrees to follow vaccination requirements set forth by Family Dog Care.

Required vaccinations include:

  • Rabies
  • Bordetella
  • Lepto
  • DHPP (distemper, adenovirus [hepatitis], parainfluenza, and parvovirus).

In addition to required vaccinations, pet(s) must provide a Clean/Negative Fecal Test prior to participation and annually thereafter.

All dogs must be neutered or spayed by 8 months of age.

Puppies under 12 weeks of age must maintain their recommended vaccination schedule provided by their veterinarian, at all times, and provide staff with vaccination updates as they occur. Prior to commencement of services, puppies must provide proof of clear fecal test results and must have received their first distemper and parvovirus shots - normally given at 8 weeks.

I understand that all pets participating in daycare, boarding, or training at Family Dog Care are required to be current on required vaccinations, however there are certain viruses and different strains that cannot be fully vaccinated against. Kennel cough, Upper Respiratory Infections, Flu, Giardia, Parvovirus, external parasites (ticks, fleas, and mange) and internal parasites and other ailments can be contracted at any time a dog comes into contact with another dog. I will not hold Family Dog Care and its staff liable for the contraction of any such illnesses or parasites during my pet’s stay.

All attending or staying at Family Dog Care daycare and boarding facilities will be checked for flea and/or tick infestation. I understand that if fleas or ticks are found on my pet, they will be required to go home or be given a flea bath or tick extraction at the owner`'s expense. While every effort is made to maintain a flea/tick free environment, I will not hold Family Dog Care or its staff liable for the occasional occurrence of fleas or ticks.

HEALTH STATUS and MEDICAL CONDITIONS: Owner will stipulate any existing and pre-existing Medical Conditions. We are only able to render services to pets who are in good physical health and free of contagious illness. If your pet(s) is showing signs of any illness, especially contagious ones (kennel cough, giardia) it is your responsibility to communicate this to Family Dog Care within 24-hours. Pets who are sick upon arrival - or become ill in our care, will be unable to be left in our care for any service at any time. If a pet becomes ill or if the state of the animal’s health otherwise requires professional attention, Family Dog Care, in its sole discretion, may engage the services of a veterinarian.

In case of emergencies, injuries and illnesses, every effort will be made to contact the Owner and their two Emergency Person(s) and follow the directions of the Veterinary Release Form. If the owners and emergency contacts cannot be contacted within a timeframe conducive to the necessary treatment - the owner hereby authorizes Family Dog Care to provide appropriate medical care by a veterinarian.

I authorize emergency medical care to be provided for my dog(s) by the veterinarian noted on the Client Registration Form, or an appropriate alternative to be determined by Family Dog Care in the event that my regular veterinarian is not available or that closer care is required.

Owner agrees to indemnify, defend and hold Family Dog Care its employees and agents harmless from loss, cost, expense arising from any veterinary treatment. I agree to reimburse Family Dog Care upon pick up of my pet for any costs incurred in such an event.

While Family Dog Care and its staff will make every effort to secure necessary veterinary care in an emergency, I will not hold Family Dog Care and its staff liable if unforeseen circumstances make it impossible for them to do so.

I authorize Family Dog Care to administer or seek first aid and resuscitative care for my dog(s) as determined appropriate by Family Dog Care and I agree to indemnify and hold harmless Family Dog Care for all and any results thereof.


Your dog will come into contact with other dogs. Dogs play with their mouths, paws, and claws, and full body weight. While dogs play and interact, they jump, lunge, run, and fall, during these activities accidents may sometimes occur. Owner understands that minor cuts, scrapes, muscle strains, injuries and bites, may happen due to the nature of being in a multi-canine, exercise, pack setting and in various environmental surroundings. Just like kids on a playground, sometimes injuries are unavoidable.

The excitement, exercise and playtime at a daycare, boarding and training environment may present itself at home. Your pet may act more tired, more hungry or thirsty. We encourage pet parents not to be alarmed. This is generally caused by the exercise and engagement they receive - both physical and mental - throughout the day.

I hereby release Family Dog Care from any liability arising from my Pet’s attendance and participation in the day care, boarding, training or group-play environment and off site play experiences. I understand that there are risks and benefits associated with group socialization of dogs. I agree that the benefits outweigh the risks and that I accept the risks. I authorize Family Dog Care and its staff to allow my pet(s) to interact with other pets at the facility.

I understand that, although all pets are evaluated for temperament and personality and play times are supervised by Family Dog Care staff, my pet(s) may become injured during interaction and play with other dogs. I agree to hold Family Dog Care and its staff harmless for damages resulting from such injury.

Owner will stipulate any Behavior Problems on the Owner/Animal Form and be responsible for physical damage done by the pet, to the kennel, transport vehicle, daycare facilities (fees start at $100.00), to himself, another pet, or a person. As a result of his behavior, should a Pet bite any employees, owners, visitors, and/or other pets, the owner is liable for all Doctor and Hospital Bills.

If Family Dog Care determines that the Client’s pet(s) is unmanageable or poses a danger to the health or safety of itself, other pets, other people or the Family Dog Care, will attempt to contact the Client to arrange alternative care. If the Client and Emergency Contacts cannot be contacted, Client authorizes Family Dog Care to place the pet in a licensed veterinary care kennel, prepared to handle such pet(s), with all charges and fees arising to be the responsibility of the Client.

Client agrees to indemnify, hold harmless and defend Family Dog Care, its owners and staff, in the event of a claim by any person injured or otherwise damaged by Client’s pet(s) or negligent act, and accepts responsibility for all medical expenses and other damages.

ALLERGIES and SENSITIVITIES: It is the owner's responsibility to inform Family Dog Care of any allergies or sensitivities to food, ingredients and products. I understand that while the staff at Family Dog Care will do their best to keep any dogs with food allergies from consuming any food(s) that they may be allergic to, there are circumstances out of Family Dog Care’s control (food dropped on floor, etc) that may result in an allergic dog consuming a food they have an allergy to.

At Family Dog Care a clean, sanitary environment is essential and vital. We use veterinary and industry-standard cleaners, products and techniques. We also use aromatherapy throughout our facility and fragrances in pet care products (wipes, shampoo/conditioner, spritzes, etc) Client understands that it is their responsibility to communicate any allergies or preferences to Family Dog Care. I understand that some dogs may have mild reactions or allergies associated with these products. On the occasion that such occur, I agree to hold Family Dog Care and its staff harmless for damages resulting from such injury.

Family Dog Care believes in returning a clean and fresh pet back to their parents. Any Pet that is excessively dirty will be bathed - this includes for boarding, daycare and/or training services. For other instances, we may use wet wipes or towels. For all boarding services, a bath is required for stays longer than 4 days. The owner is aware that they will be responsible for any charges incurred for this service. I hereby release Family Dog Care from any liability arising from my pets' allergies or sensitivities that may occur due to food, ingredients, or products used by Family Dog Care. I agree to indemnify and hold harmless Family Dog Care staff, owners, contractors, and agents collectively for any allergic reactions or sensitivities due to the use of products, treat, food at Family Dog Care.

BOARDING: If I request or approve multi-pet accommodations, I assume all responsibility for any injuries or illness they cause one another. I also authorize Family Dog Care and its staff to place each dog in a different sleeping room, suitable for the health and safety of each dog should this situation arise. Family Dog Care is not responsible for damaged or lost leashes, apparel, toys, bedding, blankets, food, treats, etc. left with the pet.

WALKS and OFF-SITE SERVICES: I authorize Family Dog Care staff to potty and take my dog out with other dogs, within the immediate property, surrounding premises neighborhood of Family Dog Care. I understand that my pet will be walked on a leash by Family Dog Care staff. I authorize Family Dog Care staff to provide off-site pet care services for my pet(s) including dog walking, hikes, dog park visits, home visits, etc. I understand that Family Dog Care will provide due diligence to ensure the safety of pet(s). I release Family Dog Care and its staff from liability for any damages, caused by persons, their collars, stray animals, fellow canines or forces of nature encountered in such time spent outside of the facility and off-site.

Transportation: I understand that Family Dog Care provides transportation services for pets in a safe manner. I understand there is a fee for Valet services outlined in our service descriptions. I understand that service availability may vary based on weather and traffic delays. Pick up and Drop off times are set within a specific timeframe. Reservations are required at least 24 hours in advance. If my Pet is transported by Family Dog Care, I will not hold Family Dog Care and its Staff responsible for any injury or accident during transportation. Due to our stringent timeline for valet pickup and other dogs in the vehicle, dogs must be prepared to leave with gear on/near the door and ready to depart within a reasonable amount of time (3 minutes), otherwise a wait fee will be charged. For drop-off service, readily available access to your home must be available. Our team will knock and call. If no response is received, the team member will have no other choice but to keep the dog for the remainder of drop-offs/return to the facility. Inconvenience fee of $15 will be charged.

PRICING TERMS and CANCELLATION POLICY: Owner authorizes Family Dog Care to charge the credit card account for costs and booking fees associated with all services: daycare, boarding, training, grooming, general pet care, and related retail products. Client agrees to pay all charges accrued for services rendered during daycare, boarding, training, or other pet care-related services. Owner authorizes Family Dog Care to charge the credit card account for any outstanding balances, late fees or cancellations, damages, veterinary fees, etc. Family Dog Care, LLC may change prices of any/all services at any time. All prices are visible on the booking platform. When possible, Family Dog Care will update clients; however, It is not the responsibility of Family Dog Care to advise of such changes. Family Dog Care reserves the right to charge additional fees for services we consider over and above the norm covered by our standard rates. If at any time the credit card does not process, is expired, or declined, services will be refused and be unable to be booked until such card is updated.

We do not accept cash or checks. No refunds for services rendered. No returns, only exchanges for retail purchases.


All Daycare reservations must be made 24 hours in advance to ensure availability. For boarding, please submit a reservation to ensure we staff appropriately. $10 inconvenience fee for Daycare no-shows. Standard daycare day is Monday - Friday [8 am – 5 pm and Saturday [9 am-5] pm. Sunday for Boarding Clients Only. Late fees apply for late pick-ups.

Overnight Boarding: Due to the nature of our boarding facility, with limited boarding, we adhere to a strict cancellation policy for all boarding services.

Holding Fee: All Boarding services - over two days - require a 50% holding fee, due upon booking services. Boarding reservations will not be reserved until Family Dog Care receives payment of the holding fee. The remainder of the balance is due at pickup.

Standard boarding stay is 8 am – 12:00 pm the following day. All clients may participate in daycare play the day of drop off and until 12 pm the day of pick up. For any pick-ups after 12 pm, you will be charged a full day of daycare.

Boarding Cancellations (non-holiday):

  • 50% Deposit on any stay over 2 days
  • 25% inconvenience fee for any cancellations less than 1 week (7-days) away from check-in day
  • Lose full deposit if less than 48 hours till check-in or no-show

Boarding Cancellations (Holidays):

  • All nightly holiday boardings require a 50% Holding Fee/Deposit
  • If we do not receive your cancellation by the morning of your scheduled check-in or if you are a no-show, the credit card on file will be charged for the reservation in its entirety.
  • Cancellations less than one week (7-days) away from check-in of services will result in a loss of the Full Holding Fee/Deposit
  • Cancellations less than 1 month from check-in will result in a 25% inconvenience fee of the total services booked


Holiday Boarding Reservations are subject to a $20 nightly premium

Day Care reservations during Holiday/Black-Out dates are subject to a ($10 upcharge)

Holiday and Black Out Dates with an up charge:

  • Christmas 2021/New Year’s 2022: Friday, December 17, 2021 – Sunday, January 2, 2022
  • MLK Jr Weekend: Jan 14-17, 2022
  • Winter Recess - Pres. Day 2022: Feb 11-22, 2022
  • Good Friday and Spring Recess: April 15 - 25, 2022
  • Memorial Day 2022: May 27 - 31, 2022
  • Independence Day 2022: Friday, July 2nd – Monday, July 5th, 2022
  • Labor Day: September 2 - 6, 2022
  • Columbus Day: October 7 - 10, 2022
  • Veterans Day: Nov 11- 14
  • Thanksgiving: Nov 23-28, 2022
  • Holiday Break: December 18 - Jan 2, 2023

Primary Owner(s) and Authorized Caretakers for Pet Transfers

Owner understands that it is their responsibility to provide the name(s) of the approved owner(s) and caretakers of their pet(s) to Family Dog Care. Family Dog Care staff will reference these names upon client pick up and drop off and understand that if another person arrives to retrieve your pet(s) they will not be released until contact is made and approved by the owner(s). I authorize Family Dog Care to release my pet(s) to the listed owner(s), caretaker(s), and on the intake form and understand it is my responsibility to update Family Dog Care with any such changes. I understand that Family Dog Care will provide their due diligence to ensure your pet is transferred to only the approved owners and caretakers. As such, I hereby hold Family Dog Care staff harmless for any pet transfer.

Client authorizes the use of pet(s) pictures and testimonials on websites, social media, and/or marketing materials for promotional purposes.

Client authorizes this contract to be valid approval for services so as to permit Family Dog Care to accept all future in person, telephone, online, mail or email reservations and provide services without additional signed contracts or written authorizations.

Family Dog Care reserves the right to refuse client service or terminate this contract at any time, for any reason.

The Term of this document applies to all pet(s) owned by the client, including any and all new pets that the customer obtains on or after the date this document was signed. I hereby certify that I am the lawful owner of the pet(s) referenced in this application. I further certify that I have read and understood the terms and conditions of Daycare, Boarding, Training, Grooming, and general Pet Care Services Agreement as well as the Policies and Procedures and Client Payment Terms. I agree to abide by all policies and procedures and accept all of the items, conditions, and statements contained therein. You hereby execute this Agreement for your Pets(s), yourself and your heirs, successors, representatives and assigns. You further attest that if you are not the owner or sole owner of the Pets(s) for whom this Guest Application has been completed, your signature is sufficient to enter into this Agreement for and on behalf of any other owner or representative. You consent to the use of electronic signatures, and agree that a true and correct copy of this document may be produced in lieu of the original Guest Application. Should a copy be produced, you agree that it is legally enforceable and does not affect the terms of the Agreement in any way. No amendments, modifications, or supplements to this Guest Application and Agreement shall be valid unless in writing and signed by both parties.
